The importance of hydroxytyrosol in protecting low-density lipoproteins (LDL) and consequently reducing the risk of cardiovascular diseases has been emphasized by the European Food Safety Authority (EFSA). EFSA concluded that daily consumption of 5 mg of hydroxytyrosol and its derivatives is desirable to achieve this physiological effect.
In 2011, EFSA issued a scientific opinion on the substantiation of health claims related to olive polyphenols and the protection of LDL particles from oxidative damage, stating among other things:

By constant procedures of analysis and quality control, the standardization of red yeast rice extract to the active substance - monacolin K, has been ensured, aiming to maintain the effectiveness of a consistent appropriate dose. During the production of red yeast rice extract, additional analyses of the raw material for citrinin are conducted.

Red yeast rice is rich in active substances called monacolins, which are actually natural statins that inhibit the enzyme responsible for cholesterol synthesis. Among all 9 types of monacolins, monacolin K is the most active, inhibiting HMG-CoA reductase (an enzyme involved in cholesterol synthesis in the liver) and is well-known in the pharmaceutical industry for treating elevated lipid levels. Compared to statins, some studies have shown that fermented red yeast rice has similar therapeutic effectiveness, whether used alone or in combination with other supplements.
